The Damaged Inquest Surveillance Device is a hero challenge that can be found in Untamed Crags eastmost, next to the Secret Spring point of interest.

Mechanics
The explanation for this hero challenge mechanics can be found within the dialogue.
- There are 5 lights, represented on the console by "
O" and "-." "O" means on and "-" means off. There are also 5 buttons. - There are 5 buttons, and pushing a button will toggle the corresponding light on or off.
- In addition, changing one light will toggle the lights to its left and right.
- If you push button 2, it will toggle light 2, as well as lights 1 and 3. Visually:
- - - - -will turn toO O O - -. - Remember, buttons toggle lights on and off, so if light 3 was on in the last example:
- - O - -, pushing button 2 will make it becomeO O - - -.
The objective is to turn all the lights off - - - - -. The starting pattern is chosen randomly among the following options:

Notes
- Engineers can skip the mini-game by tampering with the device dialogue option, solving the hero point instantly.
- This is a simplified version of the Lights Out game which uses the same mechanic across two dimensions in a larger 5x5 grid.
